Back to School Season!

The Care Clinic hopes you're having a great summer! With back-to-school and fall athletic season coming up, now is a great time to schedule an annual comprehensive Well Child Check and CHSAA required Sports Physical! A well child check and sports physical can be completed during the same visit and at a lower cost than a sports physical alone! Sports Physical paperwork is available at your SBHC.
Call (970) 710-5049 to schedule an appointment at a Care Clinic SBHC today.

What is a School-Based Health Center?

School-Based Health Centers provide quality medical, oral health, and behavioral health services right where your children are… at their school!

School-Based Health Centers (SBHC) remove the barriers of time and travel to receive healthcare for both students and parents, increasing student seat time and supporting academic performance.

SBHC Services are available for all Summit, Lake, and Park RE-2 School District Students, Staff, and Families. We accept private insurance, Medicaid and CHP+, and Summit Community Care Cards - and can help make financial arrangements as needed. Uninsured students have access to a sliding fee scale.
