Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. It is the only clinic in its 5-county service area that offers a comprehensive sliding fee scale, primary care to Medicaid and low-income, uninsured patients, and is the only safety net dental provider.
Services include diagnosis, treatment and management of acute and chronic illnesses, well-child and adult care, family planning, gynecological and obstetrical care, prenatal and perinatal services. Our clinic is here to meet the needs of all individuals in our community.
Services include universal depression and anxiety screening, drug/alcohol screening and brief intervention, individual, group, play, couples and family therapy, case management, and crisis services.
Services include dental cleanings, fillings, limited crowns, root canals, extractions, partial and complete dentures, pediatric dentistry procedures, and treatment of gum problems.
We proudly offer healthcare services to the medically underserved, uninsured, or underinsured at affordable costs. We also accept Medicaid, Medicare and private insurance.
The Care Clinic works with patients to make sure their health needs are met first and foremost.
The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

We believe that optimum well-being includes the treatment of the mind, body, and soul. That is why we use an integrated care approach, offering treatment services for emotional health concerns in addition to physical and oral health problems that our patients experience. Our health care team consists of medical, dental and behavioral health providers as well as Health Navigators, all of whom work together and with you to ensure that you get the care you need. They huddle daily to ensure that every patient gets the best care possible when they arrive for their appointment. As a new patient, you will be assigned to one of three health care teams, allowing you to see the same health professionals every time you visit the clinic.
Our rigorous Quality Assurance and Improvement program focuses on evaluating the quality of services that we provide to make sure each is effecctive. Using the extensive reporting capabilities of our Electronic Medical Records, we are able to review the health outcome data for each patient to make sure their health improves.
Summit Community Care Clinic was established in 1993 as a weekly walk-in clinic staffed by volunteers and managed by Summit County Public Health to address the need for primary and preventative health care for people who were uninsured and underinsured in Summit County. In the first year of operation, the Care Clinic provided 294 patient visits. The mission then was to provide care for all, regardless of barriers to care and the ability to pay. A lot has changed since 1993, but the mission of the Care Clinic remains the same. We are committed to providing exceptional, integrated, patient-centered health services designed to meet the health needs of all patients, particularly those who experience barriers to care, regardless of ability to pay.
Today that mission is more important than ever. With high deductibles and costs in the central mountain region that far exceed national and state averages, health care has become a challenge for almost everyone. Although approximately half of our patients have health insurance and half don’t, all face similar challenges in navigating an increasingly complex, overly costly health care environment.
