Our primary care team of medical doctors, nurse practitioners and physician assistants work with patients to provide a full range of health care services.

Our goal is to empower our patients in meeting their health goals. Our top priority is to make sure that your care is provided in the right place, at the right time, and in the manner that best suits your needs.

Because we believe that optimum well-being includes a healthy mind, body, and soul, we offer team-based treatment services for emotional and behavioral health concerns, in addition to addressing your physical and oral health needs.

Telehealth - A Convenient Way to Connect with our Providers

Our telehealth services connect you with a provider by video or phone, from the comfort of your home. With telehealth, our providers can assess your health care needs and create a customized treatment plan. Telehealth services include, but are not limited to:

  • - Follow up of labs and discussion of test results
  • - Follow-up visits for chronic conditions such as diabetes or hypertension
  • - Evaluation and management of depression, anxiety or other mental health concerns
  • - Some sick visits, which do not require a physical exam
  • - Recommendation of COVID-19 testing
  • - Prescription or refills of a medication
  • - Access to our in-house pharmacy (Mountain Cares Pharmacy)
  • - Tobacco screening and tobacco cessation planning

To schedule and attend a telehealth appointment:

  1. Call the clinic at 970-668-4040  to request a telehealth appointment. 
  2. You will receive link via email or text near the time of your appointment notifying when the provider is ready to meet. 
  3. Log in using the link and wait for the provider to start the appointment.

Medical Services

 

Primary Care

We provide high quality primary care medical services for patients of all ages. From routine and preventive care to treatment of more complicated or chronic conditions, we are here to meet your medical needs. Well-child and well-adult checks are available to patients in need of a general check-up and those with questions about health maintenance and preventive screening. We also provide focused pre-participation sports exams for students and pre-employment examinations for daycare providers and other professionals who need this type of exam.

 

Acute Illness and Injuries

Appointments for injuries and acute illnesses such as cough, sore throat, flu can generally be scheduled within 24 hours.

 

Chronic Care Management

Patients with chronic conditions such as diabetes or hypertension are offered a variety of services, including educational workshops and nutritional and behavioral counseling. Our chronic disease management program focuses on giving patients the tools necessary to understand and monitor their own conditions so that they can lead a healthy and productive life.

 

Prenatal and Post Natal Care

We provide routine prenatal care, including ultrasounds and consultations, through a collaboration between our providers and Swan Mountain Women’s Center.  Babies are delivered at St. Anthony’s Summit Medical Center.

 

Immunizations

We offer all routine recommended vaccines and immunizations for adults and children at the main clinic and School Based Health Centers.

 

Health Coaching and Education Services

Our health coach is available to support your health care goals through coaching and education services. Our coach will meet with you one-on-one, and work to connect patients with needed community resources.   

 

Pediatrics and Newborn Care

We offer specialized care options for children and newborns through our pediatric and newborn care program. Pediatric care is offered at the main medical clinic and the 6 School Based Health Centers located in Summit, Lake, and Park counties. Pediatric and Newborn care includes well-child exams, sports physicals, and routine vaccinations.

 

Referrals and Other Services

For services not available within the clinic, such as fracture care, emergency care, surgery, and specialist care, we maintain relationships with care providers throughout the region. Our staff will help to arrange referrals as needed.

 

Gender Affirming Care

We support all individuals and their individuality. Our medical providers support patients through transition as adults and support adolescents in their transition under the guidance of speciality care. We will work closely with specialty care providers.

 

COVID-19/Coronavirus

You must be an established patient of the Care Clinic for COVID testing services. If you are concerned you may have symptoms of COVID-19, you can book a telehealth appointment with a medical provider within 24 hours. If needed, a hands-on assessment, physical exam, and COVID-19 test can be performed in our respiratory assessment area. 

 

Family Planning

We offer a range of family planning services for men and women including birth control options (including IUDs, Nexplanon and others), STI testing, vasectomy, fertility counseling and pregnancy tests.

 

Senior Care

We offer a full spectrum of care for seniors including Medicare Annual Wellness Visits and Chronic Care Management provided by Nurse Case Managers.  We work closely with our partners through the senior center to help connect seniors to the services they need including transportation, home care and more.

 

Cancer Screening

We provide routine screening for cervical, colorectal, breast, lung and other types of cancer. Make an appointment today and your medical provider can guide you through which screening tests are indicated for you.

 

DOT Physicals

Some companies require their employees to have a regular Department of Transportation (DOT) physical to ensure the safety of themselves and others. For the underinsured and uninsured we perform DOT physicals on a sliding-fee scale.

 

Immigration Physicals

Immigration physicals are provided at the Care Clinic for a set fee for new and established patients.

 

Opioid Medical Assisted Treatment Program (MAT)

If you or someone you love is struggling with opioid use, we offer a comprehensive integrated program to address the behavior and medical components of this problem. Call the Care Clinic to learn more and get involved.

 

Community Resource Navigation

We understand there are many factors that impact your ability to take care of your health. Our team of community resource navigators screen for these potential barriers and work with you to provide resources that may reduce or break through barriers. Community resource navigators provide navigation on subjects such as tobacco cessation, diabetes education, stress management, substance abuse and behavioral health.  They are also able to link patients to community resources including housing, transportation, food, utilities and child care.

Ask your provider about meeting with your Care Team’s community resource navigator.

 

STI Testing / Prevention

We offer a range of STI testing and preventative services including rapid HIV testing and Pre-exposure prophylaxis (PrEP).
