Position Summary: The Summit Community Care Clinic (SCCC) Patient Services Representative (PSR) is the first point of contact with our patients, from checking in to checking out and/or rescheduling appointments. The PSR is the liaison between the Revenue Cycle partners: the patient, front desk, billing, and our clinical teams. They will ensure that appointments, patient demographics, accounts, and insurance verification is in the Electronic Medical Record (EMR) accurately. Assist patients with paperwork, consent forms, payments, and other check-in processes when they arrive for their appointment. They will be knowledgeable in all programs of the clinic.

The Care Clinic Dental Hygienist is responsible for the delivery of professional dental hygiene services to the patients of the Clinic Dental Program, working under the administrative supervision of the Dental Hygiene Director and Chief Dental Officer. The hygienist assumes responsibility for the goals of the hygiene program, monitoring progress and implementing changes that will help meet goals. Provides quality dental hygiene care to the patient population and makes appropriate referrals as needed. Performs dental measures designed to preserve teeth and prevent the spread of oral disease. Position Summary: The Integrated Behavioral Health Provider (IBHP) resides within a primary care medical clinic, acting within the model of integrated care. The IBHP works closely with primary care providers, dental providers, psychiatry, and support staff in treating behavioral health and substance use conditions to promote health behavior change. As a part of the primary care team, the IBHP will provide comprehensive diagnostic and behavioral health services including brief assessments, interventions, psychoeducation, referrals, and consultations for patients in primary and dental care clinic settings. IBHPs work from a systems and community perspective to formulate collaborative treatment plans, delivering through brief, solution-focused interventions. Position Summary: The SCCC Dental Assistant is responsible for the delivery of professional dental care services. They assist the dentists and hygienists with procedures, room set-up, x-rays, language interpretation, and ensuring an organized and infection free environment. The Dental Assistant performs specific duties as assigned by the Lead Dental Assistant, Dental Clinic Manager and Dental Provider.
The Dental Assistant must demonstrate regard for the rights of all patients and provides respectful and considerate care focused on the patient's individual needs. The Dental Assistant provides a safe and confidential environment for patients, families, and personnel. We are accepting candidates both with Dental Assistant experience and those who need to be trained. The training program is a six-month program.

The Registered Nurse is responsible for assisting medical providers and clinic assistants in the efficient and orderly delivery of medical services as well as performing related duties as assigned by the chief nursing officer. The Registered Nurse acts as a resource to the clinic assistants, front desk staff, health educators, and providers.

The Care Clinic is located in Frisco, Colorado but has satellite clinics throughout Summit County and in Lake and Park Counties. Any direct patient care position may be required to work in any location of the operations at any time if business needs arise. All efforts to accommodate employee restrictions and schedule requests will be made, but emergent needs may override primary work schedules and locations.
