Integrated, Team Based Care

At The Care Clinic, our goal is to empower our patients in meeting their health goals. Our priority is making sure that your care is provided in the right place, at the right time, and in the manner that best suits your needs. Because we believe that optimum well-being includes the treatment of the mind, body, and soul, we offer team-based treatment services for emotional health concerns in addition to physical and oral health problems that our patients experience.

 

HISTORY OF THE CARE CLINIC

Summit Community Care Clinic was established in 1993 as a weekly walk-in clinic staffed by volunteers and managed by Summit County Public Health to address the need for primary and preventative health care for people who were uninsured and underinsured in Summit County.   In the first year of operation, the Care Clinic provided 294 patient visits.  The mission then was to provide care for all, regardless of barriers to care and the ability to pay. A lot has changed since 1993, but the mission of the Care Clinic remains the same.  We are committed to providing exceptional, integrated, patient-centered health services designed to meet the health needs of all patients, particularly those who experience barriers to care, regardless of ability to pay. 

Today that mission is more important than ever.  With high deductibles and costs in the central mountain region that far exceed national and state averages, health care has become a challenge for almost everyone.   Although approximately half of our patients have health insurance and half don’t, all face similar challenges in navigating an increasingly complex, overly costly health care environment.

QUALITY ASSURANCE & IMPROVEMENT

Summit Community Care Clinic is dedicated to Quality Assurance and Quality Improvement (QA/QI) to ensure that our patients receive the best care possible. The goal of the QA/QI program is to embed a culture of continuous quality improvement that supports the improvement of patient safety, health outcomes, and long-term organizational sustainability.

Our rigorous Quality Assurance and Improvement program focuses on evaluating the quality and efficacy of our services. Using the extensive reporting capabilities of our Electronic Medical Records system, we review the health outcome data for each patient to make sure their health improves. 

Quality Standards

We accomplish this through:

  • Ensuring that each employee is trained in quality improvement methods
  • Employing the Model for Improvement (PDSA: Plan-Do-Study-Act)
  • Holding quarterly patient feedback meetings to engage in dialogue with our patients on their care experiences and how we can improve
  • Conducting ongoing patient surveys
  • Maintaining accountability to the Board of Directors 
  • Holding quarterly community-based Quality Assurance Committee meetings
 
Summit Community Care Clinic ensures quality assurance and improvement by evaluating the care that patients receive and improving it as needed.

Quality Achievements

Quality improvement is a priority for the Care Clinic. Achievements include the following:

  • Creation of a comprehensive strategic plan to guide quality efforts
  • Improved capacity of our electronic health record (EHR) to track QA/QI data and to help staff understand and utilize data to manage patient health outcomes
  • Multiple recognitions among Community Health Centers as a quality leader

Clinical Quality Measures

The clinic has established several Clinical Quality Measures that are monitored on a consistent basis.

ACCESSIBLE CARE

The Care Clinic knows that healthy people can stay in the work force, provide for their families, and contribute to our local community.

We proudly offer healthcare services to medically underserved, uninsured, or underinsured populations at affordable costs. We also accept Medicaid, Medicare and private insurance.

Patients without insurance can access services using and income-based sliding fee scale.

No patient will be denied service for their inability to pay.
