Summit Community Care Clinic changes its name and rebrands to Elevated Community Health

After more than 30 years of serving the central Colorado region, Summit Community Care Clinic has changed its name and brand identity and is now Elevated Community Health.

Along with the new name, Elevated Community Health has a refreshed logo and will launch a website in early summer. According to the nonprofit, all services, locations and care providers remain the same. The rebranding will not affect existing appointments, record or the patient experience.

A recent press release states the organization’s growth into serving areas in Lake, Park, Eagle and Grand counties in addition to Summit plays a significant role in the name change.

“We’ve grown significantly over the years, and our name needed to grow with us,” said Josh Cogdill, CEO of Elevated Community Health, in the news release. “Elevated Community Health captures both our geography and our mission — to raise the standard of care for every member of our community.”

First introduced as a free clinic, it has evolved into a full-service federally qualified health center offering primary care, behavioral health services, dental care, pharmacy and more.

“We often hear things like, ‘I used to go there when I lost my insurance,'” said Cogdill. “But the truth is, we are here for everyone — insured or uninsured. We provide the same level of care you’d expect from any private provider, and often more. As a (federally qualified health center), we adhere to rigorous quality standards, tracking and reporting data to ensure patients receive top-tier, evidence-based care.”
