Summit Community Care Clinic provides high quality primary care medical services for patients of all ages.   From routine and preventive care to care for more complicated or chronic conditions, Care Clinic is here to meet the medical needs of medically underserved families and individuals in our community.

Integrated Care
At Summit Community Care Clinic, we believe that patients are best served when health care providers treat the whole person, not just a single symptom or condition. Therefore, we practice using an “integrated” care model, combining services for both physical and emotional symptoms in one visit, under one roof. Simply put, the integrated care model recognizes that stress and other psychological problems can affect one’s health, and in turn, physical ailments can impact one’s mental well-being. Treating both together makes a successful outcome for each patient much more likely.

Helping Those with Acute Illness, Chronic Disease, and Injuries
Appointments for acute illnesses or injuries can generally be scheduled within 24 hours. Patients with chronic conditions, such as diabetes or hypertension, are offered a variety of services, including educational workshops and nutritional and behavioral counseling. Our chronic disease management program focuses on giving patients the tools to understand and monitor their own conditions so that they can lead a healthy and productive life. Well child and well adult checks are available to patients in need of a general check up and those with questions about health maintenance and preventive screening. We also  provide focused pre-participation sports exams for students and pre-employment examinations for day care providers and other professionals who need this type of exam.

A variety of affordable diagnostic tests are available at Care Clinic. Some tests with same day results  are performed in the office, including tests for Strep, influenza, HIV, H. pylori and mononucleosis infections, urine tests to identify infection, pregnancy, and impaired kidney function, and diabetes tests to monitor glucose level and Hemoglobin A1c. We also offer electrocardiograms (EKGs), pap tests, screening for sexually transmitted infections, screening tests for thyroid disease and cholesterol abnormalities, screenings tests for colon cancer, and limited ultrasound testing.  When x-rays or other radiologic testing is required, we refer patients to the most affordable facilities for imaging and follow up on results in our office.

Prenatal Care
Routine prenatal care, including ultraounds and consultations with ObGyns and Family Practice Physicians is available in both English and Spansih. Delivery is done at ST. Anthony’s Summit Medical Center by Swan Mountain Women’s Center ObGyns. All clinic patients are eligible for prenatal services. SCCC also offers newborn and pediatric care.

The Care Clinic offers vaccinations that include; Zoster, TDAP, Pneumoccocal 13 and 23, Influenza, Hepatitis A and B,HPV9, Meningococcal, MMR, Varicella and other combined children’s immunizations.  We offer vaccines to the uninsured, underinsured and insured client. Health Coaching and Education Services
Summit Community Care Clinic offers Health Coaching and Education Services to those patients who have personal goals around their health outcomes. Health Coaches meet with patients one-on-one to discuss goals, and then offer education, support and solutions to help patients be successful. SCCC believes that health outcomes are better when patients are engaged in their health care, and understand how their habits impact their health outcomes. All Health Coaches are bilingual and culturally competent.

Other Services
Finally, for services not available within the clinic, such as, fracture care, emergency care, surgery, and specialist care, Summit Community Care Clinic maintains relationships with care providers throughout the region and  our staff helps arrange referrals when needed.
