The Care Team consists of SCCC’s Patient Navigators and Health Educators. These staff members provide numerous support services for SCCC patients, and ensure patients have the resources they need to stay healthy and active in our community. Some of the services that the Care Team offers include assistance in enrolling in presciption medication discount programs, health coaching and education services, assistance with referrals to specialists and follow-up care, referrals to other social services when needed, and medical interpretation services.
Christian is from Puerto Rico and joined SCCC in 2013. He served on active duty for 8 years in the United States Coast Guard, has a bachelor’s degree in Psychology, and a Masters in Clinical Mental Health Counseling. Christian has experience working with young adults, adults, couples and families. He approaches every client with unconditional positive regard, empathy, and genuineness. In his spare time he enjoys outdoor activities and spending time with his children, his wife and his dogs. He is very proud of being part of the SCCC team and in making a difference our community.
Patient Support Services Manager
Jenniffer is from Panama. She joined the Summit Community Care Clinic in 2004. Jenniffer feels passionate about her job because she loves helping and offering support to her Latino community.
Yoli is from Bogota, Colombia. She joined the Community Care Clinic in 2008 and has a Bachelors degree in Languages from the Pedagógica University of Colombia. She enjoys swimming, rafting, fishing and crocheting. She loves kids and pets. She feels grateful for the opportunity to work at the Clinic in order to help people and make the difference in her community.
Originally from San Diego, CA, Arturo studied at George Fox University, Oregon where he obtained his Bachelors of Science in Health and Human Performance Degree. Arturo played collegiate soccer, loves the outdoors and enjoys spending time with friends and family.He is proud to be part of the Care Clinic team, where he can assist, help and support patients from the community.
