A drop-in group for Spanish speaking diabetic patients and their family members. Patients receive nutritional information, Hemoglobin A1c testing and recommendations for maintaining a healthy lifestyle while living with diabetes. Coordinated by Margarita Wong. This group meets on the second Tuesday of every month from 4:30pm - 6:30pm in the Clinic.

Diabetes Close-up

A drop-in group for English speaking diabetic patients and their family members. Group consists of various guest speakers lecturing on topics that help patients manage diabetes. Topics include nutrition, eye and foot care, stress management, and free Hemoglobin A1C tests are offered. Coordinated by Helen Royal. SBIRT

Substance Abuse Intervention and Referral for Treatment (SBIRT) is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with substance abuse disorders, as well as those who are at risk of developing these disorders. SBIRT is a add on exam provided by our Health Educators, and consists of a screening for substance abuse. SBIRT provides motivational interviewing for early intervention and referral to treatment if necessary. SBIRT is coordinated by Jenniffer Gonzalez, who can be reached at 970-668-4057.
