Summit Community Care Clinic provides high-quality primary care medical services for adults and children without insurance. From routine and preventive care, to more complicated or chronic conditions, Care Clinic is here to meet the medical needs of low-income families and individuals in our community.

Integrated Care

At Summit Community Care Clinic, we believe that patients are best served when health care providers treat the whole person - not just a single symptom or condition. Therefore, we practice using an "integrated" care model, combining services for both physical and emotional symptoms in one visit, under one roof. Simply put, the integrated care model recognizes that stress and other psychological problems can affect one's health, and in turn, physical ailments can impact one's mental well-being. Treating both together makes a successful outcome for the patient much more likely.

Helping Those with Acute Illness, Chronic Disease, and Injuries

Appointments for acute illnesses or injuries can generally be scheduled within 24 hours and are available to patients with a current Care Card. Patients with chronic conditions, such as diabetes or hypertension, are offered a variety of services, including group and individual appointments, educational workshops, and nutritional and behavioral counseling. Our chronic disease management program focuses on giving patients the tools to understand and monitor their own condition so that they can lead a healthy and productive life. Well child and well adult checks are available to patients who need a general check up or who have questions related to their health. We also can provide Sports physicals for students and Day Care Provider physicals.

Diagnostics

A variety of low cost diagnostic labs are available to Care Clinic and many tests with same day results can be performed in the office. These include testing for infectious diseases such as Strep, influenza, HIV, H. pylori and mononucleosis,as well as urine testing for infections, pregnancy, and kidney function. In addition, we monitor chronic disease treatments with tests for blood glucose, Hemoglobin A1c and lipid levels.

Other testing services we offer include:

  • tympanometry
  • hearing screens
  • fecal occult blood testing

Other Services

Finally, for services not available within the clinic, such as immunizations, prenatal care, fracture care, emergencies and specialist care, Summit Community Care Clinic maintains relationships with many other care providers throughout the area and can help arrange referrals when needed.
