How to request a copy of your Medical Records from Summit Community Care Clinic

Obtaining a copy of your SCCC Medical Record is easy. To start your request, simply download, print, complete, and sign the Records and Information Release Authorization (pdf). Fax or mail it back to us to the attention of Summit Community Care Clinic/Medical Records. 

Please be sure to sign the form. Unsigned requests cannot be processed.

Your request will be processed within 15 business days from the day it is received. Please allow a reasonable amount of time to process your request. We will either mail it to the address specified on the authorization form, fax it to the specified fax number or you may pick it up at our office after prior arrangements are made with the Office Manager.

Records may be picked up between the hours of 8:00 a.m. and 5:00 p.m. Monday, Tuesday, Thursday, Friday and Wednesday from 9:00 a.m. 5:00 p.m.
