Eligibility
Eligibility for Care Clinic services is based on income and whether you live or work in Summit County.
Care Cards: Those who are eligible to receive Clinic services receive "Care Cards". Application forms for Care Cards must be submitted to the Clinic with appropriate documentation in order for eligibility to be determined.
When to submit your Care Card application
Applications for Care Cards are accepted on a first come, first served basis on Mondays, Tuesdays, Thursdays and Fridays from 8am - 11am, and on Wednesdays from 9am - 11am. Applicants meet privately with the Patient Access Representative to receive their eligibility determination.
