We are excited to invite you to participate in the Summit Community Care Clinic’s Patient Portal! The Patient Portal is a secure web-based healthcare application that enables you to communicate with your health care providers and connects you directly to your Electronic Health Record. 

Currently through the Portal, you will be able to:

  • View your clinical visit summaries, medications, allergies & medical history
  • Review your appointments
  • Request a refill of medication
  • Review your lab results
  • Update your personal information (new address, phone number etc.)
  • Send a secure message to your provider, health coach, front desk staff or clinical staff


Please remember, you must be 18 years of age or older to participate in the Patient Portal and have a valid email address.

Ready to get started?  Visit the Clinic in person to receive your Patient Portal user name and temporary password.   Once you receive this information, click the Patient Portal link above and you will be redirected to the Portal.  Enter your user name and temporary password.  You will be prompted to change your password. Once you have completed this process, you will be able to communicate with your healthcare provider and access your personal health record online!
