Case Management Nurse- Full TimeClick here for Job Description and to apply Pay $24-$28/hour depending on license and experience Summary: The Case Management Registered Nurse will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention.  The Case Management Registered Nurse is responsible for assisting medical providers and clinic assistants in the efficient and orderly delivery of medical services as well as performs related duties as assigned by the chief medical officer.  The Case Management Registered Nurse acts as a resource to the clinic assistants, front desk staff, health educators and providers.

Credentialing Specialist – Full Time - Click here for Job Description and to apply Pay $18 – $21/hour DOE Summary: The Credentialing Management Specialist will work with all SCCC Staff, volunteers, contractors, insurance companies, private and public insurance companies to ensure proper credentialing compliance in accordance with the Credentialing and Privileging Policy as directed by Health Resources and Services Administration, Clarification of Bureau of Primary Health Care Credentialing and Privileging Policy outlined in Policy Information Notice 2001-16, Policy Information Notice (“PIN”) 2002-22 (July 10, 2002).  Comply with all Center’s for Medicaid Services (CMS) requirements and all Federally Qualified Health Center (FQHC) risk management process.

Development Director – Full Time – Click here for Job description and to apply.  Pay is DOE  Summary: The Summit Community Care Clinic (SCCC) Development Director is responsible for initiating, developing and implementing development strategies to meet the annual and long-term needs of the organization. The Development Director is responsible for all ongoing development efforts, including but not limited to; funder outreach, annual appeal, capital and endowment campaigns, donor events, fundraisers, presentations and representing the Care Clinic in community, donor and fundraising settings. This role includes: grant management, donor research and tracking, special events and project management, major gift asks, communications, relationship building and strategic planning of annual development activities. Key to this position is the ability to multitask while maintaining a clear view of how each project supports the organization’s mission.  The Development Director position is designed to develop and build relationships with the individuals and businesses, corporations, community groups and foundations that provide revenue support for the clinic to fulfill the vision and mission of the organization. The Development Director plans and executes the strategic plan for fundraising, marketing and PR efforts in coordination with the Chief Executive Officer, Development Committee and Board of Directors. The Development Director oversees  Development positions, including Grant Writer, Special Projects and others  as needed.  The Director of Development will work closely with and report directly to the Chief Executive Officer; this position serves as a key leadership team member and an active participant in making strategic decisions affecting the agency.

Our School Based Health Clinics are located in both Lake County and  Summit County schools.  The Care Clinic serves over 8,000 patients with over 28,000 primary, dental and behavioral health visits.

Patients are primarily from Summit County but also include patients seeking services from Eagle, Lake, Grand, Park and Chaffee counties. All Position information: Any direct patient care position may be required to work in any location of the operations including any Summit or Lake County site at anytime if business needs arise.  All efforts to accommodate employee restrictions and schedule requests will be made, but emergent needs may override primary work schedule and locations. Position may be located at: 360 Peak One Blvd. Suite 100  or #325 Frisco, CO; SCCC Admin Office in Frisco at 916 N. Hwy 9 or at one of our School Based Health Clinics in Summit, Park and Lake County. Salaries are commensurate with experience.  Applicants may work anytime during operating hours Monday – Friday, 34-40 hours per week, or less if part time.
