Data Coordinator – Full Time - Click here for job description and to apply online Pay is $22.00/hour DOE Summary: The Data Coordinator is responsible for organizing data from our EHR systems in a way that helps SCCC to analyze data to make good business decisions and comply with reporting requirements. The Data Coordinator helps collect and manage data so that it can be used to analyze business functions, productivity and redundancies. The Data Coordinator works to make sure that the data is collected properly, observing any legal or ethical standards are met. The position requires cross functional collaboration with all departments.
Dental Hygienist – Full Time - Click here for job description and to apply online Pay is $38-$40/hour DOE Summary: responsible for the delivery of professional dental hygiene services to the patients of the Clinic Dental Program, working under the administrative supervision of the Lead and the clinical supervision of the Chief Dental Officer. The hygienist assumes responsibility for the goals of the hygiene program; monitoring progress and implementing changes that will help meet goals. Provides quality dental hygiene care to the patient population and makes appropriate referrals as needed. The Dental Hygienist performs related duties as assigned by the Dental Clinic Manager and the Chief Dental Officer. Required: Certification by the State of Colorado as a Registered Dental Hygienist required. Current Anesthesia certification by the State of Colorado or within one year of employment. Travel is necessary for this position regularly.
Health Navigator - Full Time - Click here for job description and to apply online Pay is $17.00/hour Summary: Health Navigator will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors. The Health Navigator shall utilize clinic data to drive programmatic decision making. This person will be responsible for tracking quality assurance measures, reducing barriers to accessing care, and identifying needs of our patients and the clinic. The Health Navigator may assist in development and implementation of health education programs. They may research and develop health education materials. They will facilitate related support groups, implement health education courses and conduct outreach programs. Health Navigators will coordinate publicity for programs, events, and related activities. They may facilitate projects or activities related to attaining or maintaining accreditation, conduct or participate in research projects related to health issues, and provide skill training related to health issues. Other tasks may be performed as required.
Patient Access Representative – Full Time - Click here for job description and to apply online Pay is $16.00/hour Summary: The Patient Access Representative (PAR) is the Clinic’s first point of contact with patients, volunteers, community partners and the general public. The PAR always presents a professional manner and is a welcoming voice into the Clinic. The Patient Access Representative is responsible for the smooth operation of the Clinic’s Front Desk Operations at the Medical and Dental offices.
QI Coordinator – Full Time 32 hours per week- Click here for job description and to apply online: Pay is $23/hour/DOE summary: The QI Coordinator supports the Chief Quality and Risk Officer in carrying out activities related to the quality, risk management, and compliance programs at Summit Community Care Clinic. They collaborate with all levels and departments within the organization to carry out action items, corrective actions, and quality improvement activities. They are responsible for coordinating with the Data Coordinator to collect data related to process outcomes and reporting it in an organized and effective manner to members of leadership so they can interpret it and take action as necessary. The QI Coordinator performs assessments related to quality, risk management, and compliance, reporting findings to the appropriate staff, and follows up on actions taken to resolve the findings. They provide administrative support for operational site visits and facilitates quality/process improvement activities by organizing Plan-Do-study-Act (PDSA) cycles with relevant staff members, supporting data collection, and closing the loop. The QI Coordinator is also responsible for carrying out activities related to patient feedback.

Full time employees are eligible for purchase of discounted 75-80% medical, dental vision, life insurance, AD&D, long Term and short term disability, Aflac Accident, Critical Illness and Hospital Insurance. Full Time Providers are eligible for Continuing Education(CME) expense reimbursement.(prorated 1st year)
• Doctorate Level MDs, DOs, DDSs, DMDs, PhDs, PsyDs are eligible for up to $2,000 reimbursement for CME expenses per calendar year. (prorated 1st year)
• Advanced Providers (NPs, PAs, RDHs, LPCs, LCSWs) are eligible for up to $1,000 reimbursement for CME expenses per calendar year.
SCCC will pay Providers one(1) week of paid time (based on usual work week up to 40 hours) for CME for Doctorate and Master Level LIP providers.
All employees are eligible for a Tuition Reimbursement Scholarship up to $1000/year.

The Care Clinic uses extensive technology system infrastructure for staff with fully digital EHR (Electronic Health Record) and streamlined platforms. Our School Based Health Clinics are located in both Lake County and Summit County schools. The Care Clinic serves over 8,000 patients with over 28,000 primary, dental and behavioral health visits.
Patients are primarily from Summit County but also include patients seeking services from Eagle, Lake, Grand, Park and Chaffee counties. All Position information: Any direct patient care position may be required to work in any location of the operations including any Summit or Lake County site at anytime if business needs arise. All efforts to accommodate employee restrictions and schedule requests will be made, but emergent needs may override primary work schedule and locations. Position may be located at: 360 Peak One Blvd. Suite 100 or #325 Frisco, CO; SCCC Admin Office in Frisco at 916 N. Hwy 9 or at one of our School Based Health Clinics in Summit, Park and Lake County. Salaries are commensurate with experience. Applicants may work anytime during operating hours Monday – Friday, 34-40 hours per week, or less if part time.
