Did you know the Summit Community Care Clinic raises nearly 50% of it’s $7 million dollar budget? Through grants, foundations and donors, the Care Clinic helps to make sure every patient who needs care, receives it.
Why should I give to the Care Clinic?
The Care Clinic provides exceptional care to Summit County and surrounding neighborhoods. Without access to preventative and primary care services, many individuals would be without options for health care, creating more of a strain on the community and resources. The Care Clinic provides an important service by ensuring that all individuals who need care, receive it in a timely way.
Additionally, the Care Clinic measures a number of health outcomes, making sure that patients don’t just receive care, but are improving their overall health and well being. Outcomes are measured quarterly and a plan is implemented to continue improving each one. By providing patient centered care and allowing patients to determine and understand their own health, the Care Clinic provides exceptional integrated service.
