  • Care Clinic received its ‘Patient Centered Medical Home’ Level 3 designation in 2016.
  • Received the ‘Award for Excellence in Health and Human Services’ from El Pomar Foundation in 2013
  • Dr. Diane Leeb, Plastic Surgeon received the Volunteer Clinician Award in 2013 from the Colorado Community Health Network.
  • Featured by The Colorado Trust as one of 4 clinics in Colorado working to improve health equity for patients in 2013.
  • Summit School District – School Based Health Center was awarded the “Patient Centered Medical Home” designation in 2011, one of only 3 school based health centers in the state of Colorado to win this prestigious title.
  • Colleen Ihnken, FNP was named provider of the year in 2011 for school based health centers through CASBHC (Colorado Association of School Based Health Centers).
  • SCCC was named a top performer for WWC in 2011.
  • SCCC’s integrated care model was highlighted on a Rocky Mountain PBS special about mental health care access in the state.
  • SCCC received 2 nominations for the El Pomar Outstanding Non Profit Award in 2011.
  • Medical Director and Behavioral Health Director were invited to Washington DC to testify on the benefits of the Integrated Care Model in 2009
  • SCCC was recognized in multiple publications for their Health Information Technology efforts, as one of the first safety-net clinics in the state to implement this technology.
  • SCCC was named outstanding non-profit by the Summit Foundation Philanthropy Awards in 1998.
